Use a Form Template
Start from one of 38 ready-made forms and add it to any page from your Divi Library.
There are 38 templates in 16 categories, such as lead generation, booking, surveys and calculators.
Import a Template#
- Go to HappyDivi Form → Templates.
- Find a template and click Import.
- In the Visual Builder, add a section from your Divi Library and choose the template.
- Edit it like any other form.
Free and Pro templates
A template that uses a Pro field or setting shows a Pro badge and can only be imported with Pro.
Each imported copy is its own form, so its entries and analytics stay separate.
