The best Divi 5 form plugin depends on what your form has to do. For a simple message form, Divi's own Contact Form is enough. To keep that module and add saved entries, uploads and multi-step forms, look at Divi Contact Form Helper. For a separate native module with a free start and Stripe, look at HappyDivi Form, which we make. For user-edit forms or Zapier, look at Divi Form Builder.
This guide compares seven options side by side, shows what real Divi sites use, and ends with a simple way to choose.
How we compared
We make HappyDivi Form, so weigh our opinion of it with that in mind. We tested our own plugin hands-on. For every other option, we read the vendor's own pages and changelog on 5 November 2026 and did not test the plugin. "Not listed" means we did not find it on those pages, not that it can never be done. Prices change, so check before you buy.

How to Choose in One Minute
Stop at the first row that describes your form.
1. Divi Contact Form
Best for: simple contact forms that only need to reach an inbox.
The form module comes with Divi, so there is nothing to install. Elegant Themes' Divi 5 guide covers its email setting, spam protection, success message and redirect. Its fields also have conditional logic.
Watch for: a message depends on the email arriving. Its documentation does not list saved entries, and we explain the fix in our guide to saving Divi contact form entries. If the emails never come, start with this troubleshooting guide.
2. Divi Contact Form Helper
Best for: sites that want to keep Divi's form module and add features to it.
Pee-Aye Creative's plugin adds saved entries, file uploads, multi-step forms, conditional email routing, confirmation emails, SMTP settings and spam tools such as a honeypot and Cloudflare Turnstile. Its page says it is compatible with Divi 5 and Divi 4. It starts at $36 a year and shows a 5.00 rating from 55 customers.
Watch for: its page does not list form payments. On the plus side, it is the most widely used Divi form add-on in our scan, on 1.94% of Divi sites, so plenty of builders have tried it.
3. HappyDivi Form
Best for: Divi 5 sites that want a native form module, a free start and Stripe in one plugin.
This is our plugin. It is its own Divi 5 module rather than an add-on to Divi's form. Free includes 12 field types, 38 templates, conditional logic, saved entries, spam protection and GDPR tools on one site. Pro adds 10 more field types, including file uploads, multi-step forms and calculations, plus Stripe, CAPTCHA, the analytics dashboard, CSV export and unlimited sites, for $36 a year or $149 once.
Every submission is kept in WordPress, even if an email fails.
Watch for: it needs Divi 5, so it will not suit a site staying on Divi 4. Stripe is the only payment gateway today, and there is no built-in Zapier or CRM connection. It is also newer than the other add-ons here, so there are fewer outside reviews to read. See every feature compared.
4. Divi Form Builder
Best for: bigger form workflows inside Divi.
Divi Engine's plugin lists saved entries, conditional logic, file uploads, multi-step forms, form payments, database mapping, and Bloom and Zapier connections. Its changelog shows active Divi 5 work, including user-edit forms. Prices start at £24.99 for one site.
Watch for: an FAQ on its product page still talks about Divi 5 as a future release, so read the changelog for current status. The page does not name its payment gateways, so confirm yours is supported.
5. Contact Form Extender for Divi
Best for: adding unusual field types to Divi's form.
Cool Plugins' add-on saves submissions to your database and adds fields such as file upload, signature, calculator, rating, range slider and date picker. It was $29 a year on sale when we checked, normally $39 for one site.
Watch for: its product page says only that it works with "the latest versions of Divi". Check its changelog for your Divi 5 version.
6. Contact Form DB Divi
Best for: one job, keeping a copy of what Divi's form receives.
This WordPress.org plugin has over 4,000 active installs and says it is compatible with Divi 5 and Divi 4. Pro adds CSV export and every field, from $9.
Watch for: the free version stores only the fields with the IDs name, email and message, so other answers are not kept.
7. General WordPress Form Plugins
Best for: forms that must also run on pages outside Divi, or sites that already rely on one of these tools.
Contact Form 7, Gravity Forms, WPForms, Formidable Forms, Forminator and Fluent Forms all work independently of your theme. Divi Cake's roundup covers several of them in more detail.
Watch for: you build the form outside the Visual Builder and usually style it separately, so it may need extra work to match your Divi design.
What Divi Sites Actually Use
In our scan of 156,778 Divi sites, 27.9% had a dedicated form plugin. Contact Form 7 leads by a wide margin. Among Divi add-ons, Divi Contact Form Helper stands out, level with Formidable Forms.
Popular does not always mean right for your site, but it does show what other Divi builders trust.
HappyDivi Form does not appear in the published data, which only lists add-ons found on at least 100 sites.
Frequently asked questions
What is the best free form plugin for Divi 5?
For simple forms, start with Divi's own Contact Form, which comes with Divi. If you also want saved entries, HappyDivi Form Free adds them with conditional logic on one site. Contact Form 7 is free too, but you build its forms outside the Visual Builder.
Does the Divi Contact Form save entries?
Elegant Themes' Divi 5 guide to the module does not list saved entries, so plan on email delivery unless you add a plugin that stores them.
Which Divi form plugins can take payments?
HappyDivi Form Pro takes Stripe payments. Divi Form Builder lists form payments on its page without naming gateways, so confirm your gateway with Divi Engine first.
Which Divi form plugins support multi-step forms?
Divi Contact Form Helper, HappyDivi Form Pro and Divi Form Builder all list multi-step forms.
Do I need a form plugin at all in Divi 5?
Not always. If you only need a name, email and message and you are happy to rely on email, Divi's own Contact Form may be all you need.
